Social prescribing is an NHS service that enables health professionals to discuss and refer people to a range of local community services include art groups, gardening, befriending, cookery, healthy eating advice and a range of sports to support improvements in health and wellbeing.
The referrals generally, but not exclusively, come from professionals working in primary care settings, for example, GPs or practice nurses with the aim to address people’s needs in a holistic way.
In Sussex we have a range of ways that we provide social prescribing and we would like to hear from people who have experience of accessing these services. By sharing your views and experiences, we can initiate improvements and reduce barriers to access so more people can be supported to take control of their health.
The survey findings will be shared directly with the Social Prescribing leads and a report will be published in November 2023. The insight will support the development of the new service design due to go live in Autumn 2024 (dates tbc).
If you require the survey in paper form, an alternative language, BSL or another format please email sxicb.involvement@nhs.net
This survey will close at midnight on Friday 15 September 2023
