Health Inequalities Small Grants Project - 4Sight Vision Support, and East Sussex Vision Support

4Sight Vision Support and East Sussex Vision Support are working collaboratively across Sussex to gain vital insight for NHS Sussex. NHS Sussex would like to hear about your experience and any barriers you may face on your eye health journey. We are reaching out to seek and collect responses from 400+ people with visual impairment to get involved. These results will be fed into NHS Sussex's newly launched Integrated Care Strategy. To implement changes, we need to capture your experience by asking you to complete our questionnaire below. Your time and experience is greatly valued by us, because without you, we couldn't supply feedback into NHS Sussex. We anticipate the questionnaire taking about 30 minutes to complete.
